Racine Health Alliance Launches the 2026 Community Health Needs Assessment

Your voice matters. You know your community and your health better than anyone.

The Racine Health Alliance, a new local partnership between Aurora Health Care, Ascension Wisconsin, Racine County Public Health Division, City of Racine Public Health Department, Pillar Health, and other local providers, brings public health, healthcare systems, and community service providers together to plan, collect, and act on community health data as a team. The Alliance is urging residents of Racine County to take part in a new Community Health Needs Assessment (CHNA) survey for 2026.

This year’s survey will use responses submitted by residents from September 17 through November 20, 2026. It will take about 10-15 minutes to complete and is open to anyone 18 years and older who lives in Racine County. The survey does not ask for any personal information and responses are anonymous. No one will call to request your information, and you should not provide it to anyone claiming to represent the survey.

Survey questions cover a variety of topics, including access to health care, substance use, behavioral health, health literacy, community resources, immunizations, children’s health, and chronic diseases.  For your household and your community to be represented, the Alliance is asking residents to complete the survey and share the survey with friends, family, neighbors, and coworkers.

  2. In paper form at community events and public locations throughout Racine County (see website below).

Results will be released in the summer of 2027. The assessment provides data for ongoing evaluation of community needs and helps guide government, not-for-profit, and volunteer organizations tailor services to meet the community’s most pressing health needs. The survey is intended to:

  • Gather local data on health status, health behaviors, and risk factors in Racine County,
  • Document population health indicators related to the priorities set by the state health plan, and
  • Track data trends to evaluate the success of community health improvement activities.

Everyone’s voice is valuable. Take a few moments to complete the 2026 Community Health Needs Assessment. Your input is essential for getting an accurate picture of health priorities impacting the greater Racine community.
